[His name] has three adult children and two grandchildren from his first marriage to his late wife of 27 years. Our minor children were adopted by [His name] prior to our marriage around the time we met. The children are a sibling group that was fostered by [His name] and his first wife prior to her sudden and unexpected passing due to heart complications secondary to Lupus and Kidney disease. The two boys came into [His name]’s life at ages six and two, and their little sister joined [His name]’s family at three days old from the hospital. It took four long years to complete the adoption process before the state would release them for adoption. The children are very close, but they are siblings, and they behave as such. The children are very outgoing and full of life. They love going to the movies, eating out, and going to the park. They adore their three older siblings ([His name]’s children), and they have a great relationship with their older biological sister who is now 24 years old. They occasionally see their younger brother on holidays when we are able to schedule time.
The oldest came to live with [His name] just as he turned six, along with his younger brother. He began first grade the summer after he began living with [His name]. He loves sports and is very talkative. He is very outgoing and enjoys hanging out with friends, he wants to go into the military after high school and attend college later.
The middle child came to live with [His name] when he turned two years old along with his older brother. He adores his big brother even though they don’t always get along. He loves video games, Fortnite, Among Us, Minecraft, and Rocket League. He also likes to play basketball. He is very quiet and reserved. He would like a career in business once he graduates college. He said he wants a job working at a desk like dad because he doesn’t like to work. Lol
Our youngest came to live with [His name] at three days old right out of the hospital nursery. She is very outgoing, gregarious, and theatrical. She wants to be a medical doctor like her mom ([Her name]). She thinks she is the boss of the house when mom is not home. She likes to dress up and play with her friends. She enjoys time at the salon with mom as well as girl time getting a mani-pedi with mom. She keeps asking for a little sister or brother because she wants to be a big sister.

INTRO LETTER
Hello,
We would like to introduce ourselves to you, we are [His name], Ph.D. and Dr. [Her name]. We have been married for over three years and we have three children, two boys ages 15,11, and a little girl 8. I (His Name) work in higher education as the Director of Training and Development at a large SEC school. I am also an ordained minister currently serving as an Assistant Pastor at a Baptist church in our community. I (Her name) am a medical doctor leading a clinic with over 16 years’ experience as a board-certified Allergist/Immunologist, Pediatrician, and Internal Medicine doctor. [Her name] started a Food Pantry ministry at our church and is very passionate about supporting the less fortunate and underserved in the community.
As a family we love to read, have fun, going for walks, bike riding, playing family games, sports, and family workouts. We enjoy going to the movies almost weekly and taking family trips around the country each year. Education and exploration are very important to us. We take annual trips abroad to historical sites around the world and country. Our faith is also very important to us, we navigate life based on a Biblical world view understanding that love, compassion, and empathy are most important, as we relate to our community and the world around us.
Our minor children were adopted by [His name] prior to our marriage around the time we met. The children are a sibling group that were fostered by [His name] and his first wife prior to her sudden and unexpected passing due to heart complications secondary to Lupus and Kidney disease. The two boys came into [His name]’s life at ages six and two, and our little girl joined [His name]’s family at three days old from the hospital. It took four long years to complete the adoption process before the state would release them for adoption. The children are very close, but they are siblings, and they behave as such. The children are very outgoing and full of life. The oldest wants to go into the military, the middle child would like a career in business, and our youngest wants to be a medical doctor like her mom (Her name).
We met online through eHarmony, and from the first date we have been together almost every day over the past four years except for work-related trips and caring for our out of state parents. Commitment, honesty, and family are very important to us. We tried to conceive and have not been successful. We felt that a donor embryo was the best option for our family. It is important to us that our children know their biological siblings as well. Our children are in communication with their biological siblings that we have been made aware of.
Just a little individual background:
[His name]- I was born in upstate [State], the middle child of three. I have an older brother and a younger sister. My parents moved to the south in support of my father’s ailing parents when I was eight years old. I graduated from high school and returned to the [State] to attend college. I entered the U.S. Navy where I retired after 20yrs of service. I am a church planter and pastor. I have a B.S. in business, a Master’s in Religion, and a Ph.D. in Christian Apologetics. I enjoy music, I played trombone and drums for six years in school. I have written several songs and I am finishing the finaledits on my first book. I also enjoy playing basketball, flying drones, and model airplanes, as well as sailing remote-controlled boats.
I was married for 27 years before my first wife unexpectedly went to be with the Lord while in the hospital awaiting heart valve surgery. We had three children (now adults 30, 29, and 27) together. We lived in [City] for over 16 years. Before I retired and built a home in [other place]. All three children attended an HBCU, the oldest went on to receive her graduated degree, she teaches school and is an adjunct at her alma mater. My second daughter is also a teacher and lives [...] with her husband who is in the U.S. Navy with their two children ages seven and five. My son is a Captain in the USAF, he and his wife are expecting their first child in November.
My first wife and I were foster parents for over 10 years and fostered over 21 children before our current three join our family. Final approval for the adoption was finally granted by the judge just a few days prior to her passing.
[Her name] has been phenomenal with the children, and a great mother and support to them as they continue to grieve the passing of their mother (my first wife). The children fell in love with [Her name] the first time they met her. She and her family welcomed us with open arms. When my father passed away last month, [Her name]’s family travelled six hours to attend the memorial service. Our families have developed a close connection.
[Her name]-I was born in the south the middle child of two brothers. My mother is a retired Science teacher, and my dad is a retired higher education administrator and Chancellor of a University in the northeast. I attended undergraduate school at an HBCU in the south and medical school in Tennessee.
I practiced medicine in Miami FL, the Bay area in northern California, and in the south. I knew from a child that I wanted to be a physician. Two of my dad’s brothers were well know physicians, one of which discovered a cure a rare form of Cancer, and the other worked at the CDC before his passing. My mother’s brother is also a physician and a mentor of mine. My older brother has a master’s degree in Sports Management, my younger brother has an MBA, and his wife is a Pharmacist. My younger brother and his wife have two girls ages 11 and 8. I am very close to my family and enjoy spending the holidays with my extended family.
I am very outgoing and love to watch sports. I was a competitive gymnast as a child and cheerleader. I received a cheerleading and academic scholarship to college. I was the cheerleading captain in both high school and college.
I enjoy worship and serving in children’s ministry at church, reading, cycling, music, dancing, movies, travel, walking, working out, swimming, tennis, and yes, SHOPPING!!!
Having waited to find the perfect man to spend the rest of my life with and then be told that I was not able to bear children was a devastating blow. We considered fertility treatments, but because we both have sickle cell treat the odds of us having a child with sickle cell and other complications were too great to continue to explore that option.I have always dreamed of having a family of my own. I sacrificed having a family when I was younger in pursuit of my dream career which I now have. I was blessed to find [His name] and the children. While [His name] and the children are the family I prayed for, I have always wanted to experience the ability to carry a child to term and nurture a life from birth.
[His name] and I have a house that sits on 40 acres that we are renting out, and the five-bedroom home that we live in now with our three children. We reside in a new development in the suburbs of a large city. Our daughter attends a primary school just around the corner from our house, our mid-child attends a middle school just down the street from our church. The are currently constructing a high school were our oldest will be attending during his senior year, currently his high school is about miles away. The children’s Pediatrician is a good friend of mine, and her clinic is three miles from our house.
I believe life should be live filled with love and every child should have a home where they are encouraged and allowed to be all that God created them to be. It is our prayer that you allow us the opportunity to provide that life for our baby.

To Whom It May Concern:
I am a friend and colleague of Dr. , I am writing you in regards to and her husband's interest for embryo adoption. I've known [Her name] and her husband for approximately three
years. [Her name] is a consummate professional and dear friend. We often have lunch together at work and spend time together. She is a very compassionate and empathetic person.
I remember the time I bad to attend an all-day function and I bad their daughter babysit my three• year-old (with parental supervision). This was the first time that I have had my daughter spend time at their house. My daugther thoroughly enjoyed her family, to the point that she did not want to go home. This speaks volumes to the type ofpeople they are. They are genuinely kind and loving people, and their children are a testament oftheir excellent parenting skills.
I am aware that their children are adopted, but you would not know unless they shared that with you. The family gels together so well, and even look alike. They have two boys ages 15 and 11-years old, and an 8-year-old little girl.
In my personal and professional medical opinion, [her name] and [His name] provide a stable and loving home. They have a strong Christian faith and are family oriented. Thes have my highest recommendation for embryo adoption.
Please do not hesitate to contact me ifyou need any further information regarding their family. Sincerely,

REFERENCE LETTER 3
Hello,
I'm writing to share my strong recommendation for [His name] and [Her name] in consideration for embryo adoption. I've known [His name] most of my life, we lived on the same street and attended high school together. Growing up in the neighborhood he was always that "good kid", though not perfect at all. Lol
After high school he went away to college and then the military and we lost touch.
When he retired from the military he returned home and started a church. I attended the church he pastored, and I continue to attend after he and his wife moved out of state for job reasons. He was a great pastor and an awesome friend. He is kind and compassionate, willing to help anyone in need. He comes from a great family; I maintain the yard for his mother, and his rental property while he is out of state. His brother and my brother are good friends and have been in business together.
He is a godly man with high moral and educational standards. He seems to be a great father and has a good relationship with his children. He has three adult children and three minor adopted children. His two adult daughters are teachers, and his son is an officer in the Air Force. [His name] has always been active in the community and was a friend and consultant to the past two mayors of our city. He worked with the Boys and Girls club as well as a professor at the college teaching business and religion.
[His name] has my highest recommendation as an adoptive parent.
